Hard to say that Donald Trump has really gone off the rails, when it's debatable whether he's ever truly been on them in the first place.

Donald Trump said Wednesday that President Obama "founded" the Islamic State in Iraq and Syria (ISIS).

“ISIS is honoring President Obama,” he said during a rally in Fort Lauderdale, Fla. “He’s the founder of ISIS. He founded ISIS."

“I would say the co-founder would be ‘Crooked’ Hillary Clinton,” Trump added of Obama’s former secretary of State and his Democratic rival.
Trump criticized Obama’s decision to withdraw U.S. military forces from the Middle East and leaving behind a void ISIS could fill.

This was an intentional statement, intended to "other" President Obama and paint his political opposition as enemies of the state. From AlterNet earlier this year:

Trump’s growing support among right-wing Americans is an indication of how popular it is to believe an easily disproved lie: that blacks and Muslims are the perpetrators of terror, rather than victims of it. His dissemination of racist propaganda and his policy positions echo the ugly rhetoric of Nazi Germany. Demonstrating his clearly low opinion of Muslim Americans, he endorsed the idea of registering them in some kind of database, saying, “We’re going to have to do things that we never did before. And some people are going to be upset about it.”

Meanwhile, the NRA continues to be Trump's biggest backer, with multi-million dollar ad buys in multiple states. And the ad they're running?

On July 28, the NRA released an online ad targeting Clinton with the same false attack on her protection by the Secret Service. That ad included stills that indicated who was part of Clinton’s security detail:
