Now They Don't Even Pretend. Fox Fabricates 'Death Book' for Vets

Via Media Matters. This is even more shocking than the usual misleading Fox News coverage. This is an outright lie, manufactured by someone treated as credible by the rest of the media. But don't worry, I'm positive it will be covered in depth on all the other networks! (Not.)

On Fox News Sunday, Chris Wallace repeatedly cropped quotes from a Veterans Health Administration (VHA) document to falsely suggest that the Obama administration is pressuring veterans to end their lives prematurely and to accuse Assistant Secretary of Veterans Affairs Tammy Duckworth of lying about it. In fact, contrary to Wallace's false assertions, the document he referred to does not require doctors to direct veterans to what conservatives have labeled the "Death Book for Veterans."

WALLACE: If I may, Ms. Duckworth, because we have limited time here --

DUCKWORTH: Sure.

WALLACE: Why would a question -- I can understand questions about if you're in an irreversible coma, do you want us to pull the plug. But why -- as I asked Mr. Towey -- why would you even have a question in an end-of-life plan-- counseling book about, "If you're in a wheelchair, if you're living in a nursing home, does that make life worth living?"

DUCKWORTH: Well, I know that before I was injured, I certainly let my husband know that, you know, being in a wheelchair was something that I feared and I was afraid for, but that I also wanted my life prolonged. This is a tool. This is a simple tool that was put into place, as I said, under Secretary [Jim] Nicholson. It was something that was used throughout -- and let me make a correction. We've not used it since 2007, when under the Bush administration, we decided to go ahead and revise it.

This checklist is still under revision on a timetable according to a program that was actually decided on under the previous administration. Because I know that President Bush and Secretary Nicholson and Secretary [James] Peake all valued life. And that's what we're doing, is we're revising it. It will be out in 2010. It's not yet out. So Mr. Towey was not correct by saying --

WALLACE: Secretary Duckworth, that's just not true. The VHA put out a directive on July 2, 2009, and I want to put up two pages from that directive. The first one, Page 8. "Primary care practitioners are responsible for giving patients pertinent educational materials, e.g., refer patients to the 'Your Life, Your Choices' module." And on Page 9, it says if they request more information, "patients may be directed to the exercises in 'Your Life, Your Choices.' "

So, as of July 2, 2009 -- last month, more than a month ago -- VA health practitioners were told to refer all veterans -- not just end-of-life veterans, but all 24 million veterans -- to this document, "Your Life, Your Choices."

DUCKWORTH: Let me make a correction there, Chris. What our practitioners were told is to refer patients to any type of a tool. They can use Mr. Towey's if they want to spend the $5 apiece. VA simply was not willing to buy his booklet at $5 per veteran at the time.

WALLACE: But how do you explain --

DUCKWORTH: This is a decision that was made by the previous administration.

WALLACE: It doesn't say, "Give them access to anything." I mean, in the specific VA booklet -- it's only about 15 pages long -- it specifically refers to this booklet twice.

DUCKWORTH: The only directive that is out there is actually left over from -- I believe it was beginning of 2007, our advanced planning directive put into place, and it is actually very clear in saying that we need to provide veterans with information that they can make on how they want their care.

You know, this ultimately is about the care and health care for veterans, and we're actually expanding benefits for veterans. We're actually trying to get more priority aid veterans to sign up for VA benefits. We estimate it will be another 500,000 veterans coming in for benefits. We are expanding VA benefits to veterans. It is in our best interest to make sure that every veteran out there receives the care and access to the treatments and everything that he needs. Because after all, these are the men and women who fought for us and fought -- and they deserve nothing less than the best care.

DUCKWORTH: Chris, it has not been reinstated. Let me make it clear: The only advanced directive that we have is dated February of 2007. And it actually encourages veterans to use any type of tool or checklist they would like to. There are many, many good ones out there. This "Your Life, Your Choice" [sic] is widely used out there, not just within VA. There are many others that are out there. And veterans are free to use whatever they would like to use. We just urge them to use them.

WALLACE: Let me -- I mean, I just -- I have a problem here. And often -- too often on these shows, we just say, "One person said, and another person said." Secretary Duckworth, I don't know if we're able in the control room to put up the full screen of the VHA directive, but I'd like you to put up the first full screen if you can.

In the VHA directive of July 2, 2009, it says the following on Page 8: "Primary care practitioners are responsible for giving patients pertinent educational materials, e.g., refer patients to the 'Your Life, Your Choices' module." I mean, it's just there in black and white on the VHA directive of July 2.

DUCKWORTH: Chris, I'm sorry. I can't see that on a monitor in here. I will tell you that I know for a fact that the only directive signed by Secretary [Eric] Shinseki -- I mean, signed by a VA secretary -- was actually signed by Secretary Nicholson, and it's dated February of 2007. What you're looking at may not actually be a directive. So I -- since I can't see it -- I'd be happy to come back and discuss it with you.

Even with all the lies, Fox News still couldn't convince enough people in the '06 and '08 electiions.

People watch Fox News for the same reason they slow down their vehicles to view an accident scene.

And once again, people watch Fox News for the same reason they watch primates at a zoo throwing sh## at each other.

Frankly, she should have sent a Medical Director, a physician, who would know the ins and outs of this.

She was refuting an argument that is really beside the point about the dates of the document etc.

If the Medical director had been available, he could have shot down all of Towey's BS based on the merits of his arguments, which were prescious few.

As an intensive care physician and EOL and advance directive expert, I just skimmed through the "Your Life Your Choices" document in question. It's about 50 pages and, the only line that I thought was just a bit iffy was a question about how you would feel if you couldn't "shake the blues."

If this document is shared with a physician early, this is a very good question, as our antennae will go up and make us focus on looking for and treating depression in this person. I can see how a non-physician, like Towey, might not get that and think it is some pseudo-hypnotic way of telling a person that depression makes life not worth living. But that is an ignorant viewpoint.

The other 50 pages are exemplary however. They go into great detail about possible EOL scenarios and flesh out the person's wishes and preferences, which is what we physicians and families desperately want to know when the time comes. It is very similar to what every hospital in the country gives to patients when they enter the hospital, only more detailed.

It is a fine document and should be treated as such and be made more widely available, not less.

The right to die on your own terms is exactly the opposite of having someone else choose for you. The reason people want to have advance directives is so their wishes will be honored.

Lately, in my ICU, I've seen some patients prescient enough to indicate on their ADs that these are their wishes and that their family is not allowed to over ride, them. They have probably seen a friend suffer in an ICU for days or weeks because the FAMILY couldn't accept the end of life. You'd be amazed how brave people are when they know the end is near. Kubler-Ross's stages are for real. Acceptance is so important so that we can be at peace with our loved ones, make our arrangements.

One final word, to those who complain they don't want to think about their deaths and how they will die: If you are over 18, grow the heck up!

I work in an ICU and with each new admission we ask if they do or don't have a Living Will or Advance Directive. If they don't we then ask them if they would like information on how to create one. I've seen some fine documents that outline exactly what they want, down to the pajamas they would like to be placed in. Everyone should have the opportunity to outline how they would like the end of his/her life to proceed. It's not morbid or maudlin and makes it quite clear as to what you want and takes the decisions out of the hands of loved ones that will be dealing with many other issues and emotions.

I tell families all the time that knowing what their loved one wants is a gift to them.

Not knowing is so hard on families.

And, if you so choose, your advance directive can say to continue treatment until they're nailing your coffin shut.

and you have to document whether or not you asked and gave them the relevant information. this is not new stuff--unless youre incredibly ignorant--as FoxWatchers would have to be.

Yes, these Long Term Acute Care facilities (LTACs) are big business, but they do provide a valuable service.

They allow us to move patients who were in prior years stuck in an ICU to move out to a less expensive care setting.

Also in fairness, they have a decent success rate in getting people off the ventialtors by applying lots of rehab, nutritional support, etc. And sometimes, it gives families a bit more time for decision making.

And no, I do not have any financial interest in any LTAC.

...when I look at a patient's medical records at the medical facility consistently rated first or second in the country, the place where ex-presidents' like, say, Ronald Reagan are and were treated; one of the first things that I look at in the patient's medical record is whether or not he or she is "full code". EVERY patient MUST commit to an end of life protocol, so that the facility knows what to do in the event something goes wrong. Full code means that the physician should do everything possible to keep the patient alive, a DNR would direct the physician not to bother attempting to resuscitate. This notion that the doctor or the facility or some nefarious panel would make these end of life decisions is completely false. FOX, as usual, should be ashamed---but I doubt their Australian owner, Rupert Murdoch or the GOP operatives running the network are capable of shame.

ALSO, what is truly dangerous about this asinine Republican/FOX attack on Health Care Reform (HRC) is that if the patient doesn't make these decisions ahead of time, someone else (maybe a family member, maybe a physician) will be forced to make the call. My father, despite his clear wishes and living will, actually had to argue with his Catholic right-to-life doctor to force the physician to allow him to die with dignity. And, ultimately, that is what end of life issues are all about---a person's right to die with dignity. I don't know about the Republicans and the people at FOX News, but I want to be in charge of my life and end of life decisions and choices. By falsely making this issue into "death panel" nonsense, these Republicans/FOX are actually forcing grandma to abdicate HER WILL to that of some loon like my father's former oncologist---people, who are so blinded by their false understanding of Christianity that they feel qualified to impose THEIR WILL on the sick and dying. That, to me, is vulgar and vile and disgusting!

One more thought for the Repubs/FOX News: I am eternally grateful that my father made the decisions about when his life should end. The reason is that no one in my family, today, has to live with the decision and forever wondering if they made the right call and at the right time, which is something I had to go through with my dog of 15 years. Let me tell you, that kind of second guessing is emotional hell. People, Republicans and FOX, had better grow-up and soon! They are REALLY being extremely devious and their action will lead to a great deal of pain for those dying and the people, who are forced to make end of life decisions for them.

11,000 veterans who had colonoscopies at U.S. Veterans Affairs hospitals may have been exposed to hepatitis B, hepatitis C and HIV.

This week it was learned a 55-year-old North Miami man, Juan Rivera, a thirteen-year Army veteran with a wife and 5 children, filed notice last month that he will sue the Federal Government claiming he was infected with HIV during a colonoscopy at the Miami Veterans Administration hospital. He had a colonoscopy at the hospital in May last year.

Have we seen ANY teabaggers on Faux protesting about how Bushco tried to murdered veterans due to shoddy health care in the VA.

With a real single payer NHS system, the VA would have no reason to exist in the form it does, it could be abolished and or absorbed into a NHS system, clean out the lame political appointees and put health care professionals into their jobs.

Following congressional inquiries, Miami VA hospital director Mary Berrocal disciplined up to 10 employees.

Everything solved and fine and dandy, nothing to see now people, move along.

We are lucky that we have people like Mary in the VA to prevent possible horror stories like this ever happening, oh wait she didn't prevent anything, she just did a CYA after the fact to save her own job, benefits and pension !

Maybe Mary or her orifice minions should have ordered those flash steam sterilization units, rather than just buying and using S/H plastic buckets from local garage sales.

If I wasn't a high risk patient for that, I'd think about passing on it.

I've had one, and they found a pre cancerous polyp.
They dug it out and cauterized the wound.
I was fine. Although, I had them knock me out.
When I came too, I was fine. A little discomfort but nothing major.
